At Stepping Stone Pediatrics, we want to make your visit as hassle-free and expeditious as possible. Although we provide copies of forms for you to complete in the office, you can expedite the process by completing the New Patient Registration prior to the visit. New Patient Registration
The Stepping Stone New Patient Registration form is designed to provide our medical staff with a complete picture of your child’s health. These forms include pertinent information about any healthcare your child has received outside of Stepping Stone Pediatrics as well as a request for transfer of records from previous providers to help provide a thorough medical history.
By collecting this information, Dr. Kim and his medical staff will be able to provide your child with the highest possible level of care. You can either print this form and complete it before your visit or complete it in the office at your first visit. If you chose the latter, we do ask you arrive a few minutes early to spend an adequate amount of time on the form.

School Forms (3231/3300)
If your child needs forms for school to document immunizations or hearing/vision screening, just let us know. We are happy to provide these to you at your child’s Well Child Care visits or upon request. Please give us one business day to process this paperwork.
If you call to request one of these forms (or almost any other paperwork), you can pick them up in our convenient Drive-Through window. No more having to unbundle sleeping children to trundle up to the reception desk!
Forms for Services (Katie Beckett, FMLA, SSI, etc.)
Some families require forms that are require more detail. Examples of these are such things as Prior Authorization Forms for prescriptions, Katie Beckett Forms, Disability claims, Family Leave requests to name a few. Because of the need to review your child’s individual history for such paperwork, they require a bit more advance notice. Our staff will provide you a time frame for expected completion when you call with your request. Rest assured that we will do our utmost to return the completed forms to you as quickly as possible.
